Terms and Conditions Agreement for New Patients at G&G Optometry & Optical

Introduction

Welcome to G&G Optometry & Optical. These terms and conditions ("Agreement") govern the relationship between you, our patient, and G&G Optometry & Optical. By registering as a new patient, you agree to comply with and be bound by the terms of this Agreement.

1. Services Provided

G&G Optometry & Optical provides comprehensive eye care services, including but not limited to eye examinations, optical coherence tomography (OCT) scans, prescription of eyeglasses and contact lenses, and treatment of various eye conditions.

2. Appointments and Cancellations

2.1 Appointments
All appointments must be scheduled in advance. We recommend arriving 10 minutes prior to your scheduled time.

2.2 Cancellations
If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Failure to do so may result in a cancellation fee.

3. Payment and Insurance

3.1 Payment
Payment is due at the time of service unless other arrangements have been made in advance. We accept various forms of payment including cash, credit/debit cards, and approved insurance plans.

3.2 Insurance
It is your responsibility to provide accurate and current insurance information. We will bill your insurance as a courtesy, but you are responsible for any co-pays, deductibles, or non-covered services.

3.3 Additional Services
Some services, such as OCT scans, may not be covered by your insurance. You will be informed of any additional costs before the service is provided, and you must agree to these charges before proceeding.

4. Privacy and Confidentiality

4.1 Privacy
Your privacy is important to us. We adhere to all applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

4.2 Confidentiality
All patient records and information are kept confidential and will not be disclosed without your consent, except as required by law.

5. Patient Responsibilities

5.1 Accurate Information
You agree to provide accurate and complete personal, medical, and insurance information.

5.2 Compliance
You agree to follow the treatment plan prescribed by your optometrist and to adhere to the recommendations for follow-up appointments.

6. Limitation of Liability

G&G Optometry & Optical is not liable for any indirect, incidental, or consequential damages arising from the use of our services. Our liability for any claim related to the services provided shall not exceed the amount paid by you for those services. We are not responsible for damages, including loss or theft, to your glasses or other personal belongings.

7. No Refund Policy

There is no refund for services provided or products purchased at G&G Optometry & Optical.

8. Dispute Resolution

Any disputes arising from or related to this Agreement shall be resolved through binding arbitration in accordance with the rules of the Canadian Arbitration Association.

9. Glasses Handling Disclosure

While we take great care in handling all eyewear, G&G Eye Doctors, Optometry & Optical (Cambridge) is not responsible for any glasses that are lost, stolen, damaged, or broken while in our possession, whether for adjustments, repairs, or any other reason. By leaving your eyewear with us, you acknowledge and accept this policy.

10. Amendments

G&G Optometry & Optical reserves the right to amend this Agreement at any time. Notice of any changes will be provided to you, and your continued use of our services constitutes acceptance of the amended terms.

11. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the province in which G&G Optometry & Optical operates.

By signing, you acknowledge that you have read, understood, and agree to the terms and conditions outlined in this Agreement.
